Transition Planning Meeting

A Transition planning meeting takes place as part of the Annual Review of the Statement of Special Educational Needs. 

A number of people will be invited to take part in this meeting and contribute to drawing up the Transition Plan.  This would include the child or young person and their parent or carer. 

Other people who may be in attendance could be:

  • A school representative, for example, the SENCO
  • A careers adviser
  • The Education Transition Co-ordinator
  • A representative from the Health and Social Care Trust
  • Any other person who make be able to make a contribution to the planning process.

As with many things, preparation is the key to success which is why the Transition Service, along with the School, places great importance on preparing a young person to play as active a role in the meeting as possible. 

We know that some young people prefer not to attend the meeting.  If this is the case it will be important that they are given the opportunity, prior to the meeting, to record their views. 

In preparation for this meeting, your child will have the opportunity to complete a pupil contribution form.   You can help them with this, or they can ask for help from the SENCO or the Transition Co-ordinator.  Your contribution is also very important and the school will provide you with a parent contribution form.  If you are worried that you will be unable to express your views, you can ask for help from the Transition Co-ordinator. 

Some issues you may be asked to clarify are, for example, does your child have a social worker, what are your child’s independent traveling capabilities like or is your child presenting with any new medical, learning or behavioural needs.

This dialogue allows the Transition Service to ensure that the Transition Plan is tailored to meet your child’s individual needs.

At the final Transition planning meeting you will be asked to sign a Consent Form.  This will allow the Transition Service to share the Transition Plan with other bodies and agencies.  By doing this we can ensure that services have the information they need determine what supports can be put in place for your child when they move on from school.
