School Crossing Patrol - Survey Request Form

This form MUST be completed by the Principal.

Important Information

Schools should be aware that:

  1. This request must have the endorsement of your Board of Governors.
  2. Surveys will not be repeated within a 2 year period, unless there is evidence of significant change as outlined in the EA’s SCP Scheme document.
  3. There may be some factors which preclude the operation of a SCP, such as a 40mph speed limit or light controlled crossing.  In these circumstances EA may not proceed to a full site survey.
  4. An exact location must be provided in order for a survey to be conducted.  Schools will be asked to upload supporting documents

Please note the supporting evidence and the additional information provided by the school in response to the survey request form should not include information relating to identifiable individuals (e.g. the faces of any individuals appearing in photographs of the existing or proposed School Crossing Patrol location should be obscured to ensure that they are not identifiable, and whilst the additional information section may include information about problems experienced by children and parents, the children and parents should not be identified).
